What We Do

Alliance Home Health Care is a Medicare-certified home health care agency, providing skilled nusring and therapy services at patients' homes.

We take pride in providing high quality care to our patients. Our patient  surveys consistently show a very high rate of patient satisfaction.

One Simple Phone Call

One simple phone call followed by a fax is all you need to make a referral.  A physician's order is required to receive skilled home care services.  The referring physician's office staff or the hospital discharge planner can make the call after the physician places the order for home care.

Individualized Treatment Plans

Within 24-48 hours of receiving a referral a comprehensive assessment is performed to identify the patient’s needs.  Under the direction of a physician and in collaboration with the patient, family members and other caregivers, a treatment plan is designed to meet the patient’s individual needs.

Each patient is under the supervision of a skilled discipline such as a nurse or therapist. Each patient is assigned a case manager to ensure continuity and co-ordination of care to provide skilled nursing, rehab, medical social services and home health aides, as needed. Multidisciplinary meetings are held to discuss and coordinate patient care. Our staff serves as a liaison between the patient, the family/caregiver and the physician.
