We provide skilled nursing and therapy services to home-bound patients.
Medicare Definition of Homebound
Medicare considers a patient homebound if leaving the home would require a considerable and taxing effort and requires the aid of assistive devices or assistance from another person.
Services provided
Skilled Nursing
Educates patients to enable them to manage their condition or disease process at home. Skilled care includes pain control, symptom management, medication instruction and utilization of resources.
Rehabilitation Staff(Physical Therapy, Occupational Therapy and Speech Therapy)
Provides comprehensive training and education to patients and caregivers to maximize their potential for functional activity and mobility. Interventions may include recommending adaptive equipment and/or home modification.
Medical Social Workers
Evaluate patient and home environment in order to identify barriers to recovery. Refers to appropriate community agencies to meet continuing care needs.
Home Health Aides
Assist with personal care, including bathing and grooming activities, under a care plan established by a licensed clinician.
